1. Consent 1.1. Parental Consent for Minors: (Herein after "Parent" refers to a parent, guardian, or carer who has parental responsibility for the competitor). If the competitor is under 18 years of age, I, as the competitor’s Parent, confirm that the competitor has my authorisation to participate in this British Judo event. 1.2. Personal Consent for Adults: If the competitor is 18 years of age or older, I, as the competitor, confirm my own consent to participate in this British Judo event. 1.3. Health and Participation: I (or the Parent if the competitor is under 18) further confirm and warrant that, to the best of my knowledge and belief, the competitor is fit and able to participate in the Judo event. I understand that the event may be live-streamed, and I acknowledge that there is no expectation of privacy regarding the live stream. British Judo may use this live stream for promoting judo or for applying for funds to support these purposes. 1.4. Medical Authorisation: If a medical emergency arises during the competitor’s participation in this event when I (or the Parent if the competitor is under 18) am not present, I hereby authorise the Competition Organiser and Chief Medical Officer to take necessary measures to ensure the competitor receives any emergency medical treatment, including hospitalisation, deemed advisable to protect the competitor’s health and well-being. 2. Declaration of Consent - Competitor Aged 16 or Over 2.1. Media Consent: I consent to the following uses of my image, likeness, and details: • My photograph, video and audio recordings may be used by British Judo for display purposes, on their website, and on social media pages, in the promotion of judo. • My photograph and video may be used in other printed publications in the promotion of judo. • My video may be used for training or analysis purposes. • My personal details (including full name, weight, age category, club name, and Area/Home Nation) may be used in pre-event entry lists and event results, both online and on social media. 2.2. Data Sharing Consent: I consent to my personal details being shared with the competition organiser. 2.3. Awareness of Media Storage: I confirm that I have read, or been made aware of, how these details, images, videos or audio recordings will be stored within British Judo 3. Declaration of Consent - Parent/Carer of Competitor Under 16 3.1. Media Consent: As the parent/carer, I consent to the following uses of my child’s image, likeness, and details: • My child’s photograph and video may be used within British Judo for display purposes, on their website, and on social media pages. • My child’s photograph and video may be used in other printed publications. • My child’s video may be used for training or analysis purposes. • My child’s personal details (including full name, weight, age category, club name, and Area/Home Nation) may be used in pre-event entry lists and event results, both online and on social media. 3.2. Data Sharing Consent: I consent to my child’s personal details being shared with the competition organiser. 3.3. Awareness of Media Storage: I confirm that I have read, or been made aware of, how these details, images, videos or audio recordings will be stored within British Judo 4. Email and Contact 4.1. Communication: Contact will be made using the details provided in this form submission, including pre-event, in event and post event information. If you do not wish to be contacted regarding this event or future events, please contact the events team. 5. Prohibited Activities 5.1. Restricted Areas: Participants, parents, guardians, and spectators are prohibited from taking pictures in changing rooms, toilet areas, and during the weigh-in portion of the competition. 5.2. Social Media Restrictions: Photographs taken for personal use must not be posted on social media if doing so may adversely affect participants, particularly where there are safeguarding concerns. 6. Disclaimer and Agreement 6.1. Media and Live Stream: A dedicated media team will be present at the event, and the event may be live-streamed. Those conducting the filming and photography will have undergone the necessary checks. By entering the event, all participants and their parents or guardians consent to photography, video and audio recording by British Judo or an official contractor, with no expectation of privacy. These photographs, videos or audio recordings may be used in future British Judo social media, publications, or by contractors to promote the sport. 6.2. Opt-Out: If you wish to opt-out of the above, please contact events@britishjudo.org.uk or speak to a member of BJA staff at the event. 7.
